Grant Qualification Criteria for NJ Smiles

✔ Must be a resident of New Jersey (and/or surrounding NYC Boroughs)

✔ Must demonstrate financial need (based on income level or financial hardship)

✔ Must have significant tooth loss or dental issues that impact health and well-being

✔ Must have limited alternative means to cover dental implant costs (e.g., insurance, savings, or other financial assistance)

✔ Must submit a dental evaluation from a licensed professional recommending implants

Additional Considerations

🔹 Priority given to individuals experiencing severe medical complications due to missing teeth (e.g., difficulty eating, speech issues, jawbone deterioration)

🔹 Special consideration for veterans, elderly individuals, or those affected by domestic abuse, homelessness, or medical conditions leading to tooth loss

🔹 Willingness to participate in follow-up testimonials or case studies to inspire future donors and applicants
